Provides home based practical relief and support for the Carers of anyone of any age were a physical or mental disability, including frail elderly. Breaks can be regular or on an occasional basis. The service is arranged flexibly around each families individual needs. |

Based at St. Oswald’s Hospital . Partnership project involving the Community Health Services NHS Trust, General Practitioners, Social Services, Age Concern and Ashbourne Volunteer Bureau. Offers daily telephone call to check all is well to people referred to the scheme by open referral (who are considered vulnerable, isolated or at risk) . To ensure that any difficulties are dealt with and that crises are prevented. |
